Eye/Ear Antiseptics with Corticosteriods

EYE : Initially, instill 1 or 2 drops into the conjunctival sac of the affected eye(s) every one to two hours. When a favorable response is observed, reduce dosage to 1 drop every four hours, later further reducing to 1 drop three to four times a day to control symptoms. EAR: Clean and dry aural canal thoroughly. Initially, instill 3 or 4 drops into the aural canal two or three times a day. When a favorable response is observed, reduce dosage gradually and eventually discontinue.

Dexamethasone Sodium Phosphate equivalent to Dexamethasone Phosphate 0.1 % w/v
Neomycin Sulphate 0,5 % w/v
Phenylmercuric Nitrate as preservative. 0.002 % w/v

Dextracin Eye-ear Drops should not be used in the presence of acute herpes simplex, tuberculosis of the eye, fungal diseases of ocular or auricular structures, vaccinia, varicella and most viral diseases of the cornea and conjunctiva. It is contraindicated in perforated drum membrane, and in patients sensitive to any of its components

Prolonged use may result in glaucoma, damage of the optic nerve, defects in visual acuity and fields of vision, posterior subcapsular cataract formation or may aid in the establishment of secondary infections. Use in pregnancy should be avoided, unless absolutely necessary and under close medical supervision. Avoid use in nursing mothers as corticosteroids may appear in breast milk. In those diseases causing thinning of the cornea, perforation has been known to have occurred with the use of topical steroids. As with other antibiotic-containing preparation, prolonged use may result in the overgrowth of non-susceptible organisms. Aminoglycosides antibiotics may cause irreversible, partial or total deafness when applied topically to open wounds or damaged skin. This effect is aggravated by renal or hepatic impairment and by prolonged duration of treatment. The treatment should not be continued after resolution of symptoms. Visual disturbance may be reported with systemic and topical corticosteroid use. If a patient presents with symptoms such as blurred vision or other visual disturbances, the patient should be considered for referral to an ophthalmologist for evaluation of possible causes which may include cataract, glaucoma or rare diseases such as central serous chorioretinopathy (CSCR) which have been reported after use of systemic and topical corticosteroids.

Allergic local reactions to neomycin are usually serious only in patients hypersensitive to aminoglycosides or with a history of allergy. Discontinue treatment if hypersensitivity reactions occur during use. Blurred vision may occur.

Topical application of Dextracin Eye-ear Drops is not likely to produce significant drug interactions. Systemically, neomycin may enhance the anticoagulant effects of warfarin and nicoumalone. Its ototoxicity may be increased by ethacrynic acid and frusemide. Concurrent use of barbiturates, phenytoin or rifampicin may enhance the metabolism and decrease the effect of corticosteroids.
